First-Time Patient Forms

For your first visit please bring with you:

• A completed version of the new patient intake
• Current or recent prescription medications, ideally in their original bottles
• Recent lab results and/or genetic testing reports (or have them faxed to us)

Please fill out, sign, and bring with you to your first appointment.

Recurring Visit - Patient Form

Each visit bring the Patient Questionnaire. Please download and fill out the form prior to every appointment with Dr. Cole.
